| Job Description: | Looking for Support Workers to join our team. Your role will include but not limited to; 1. Helping people plan and organise their lives in ways that they choose. 2. Helping people to stay healthy and well. 3. Helping people to live safely in their own home. This includes keeping their home clean and safe, supporting them to pay bills and to make choices about daily living. 4. Supporting people who find it hard to be part of their community. This may include using transport, building relationships and using the local shops. 5. Supporting service-users with shopping and daily activities 6. Completing hand over notes and implementing Care plans |
